Accessing one specialty pharmacy patent's PHI on another client's computers.?

Is it a violation of HIPAA if an employee of a software vendor who services specialty pharmacies to access one client's patient's and PHI on another client's computers? I know someone who was at a client site, and accessed another client on the computer and worked patients through. No, neither client was aware.
